REQUESTING YOUR DD214

There are four (4) ways to request your DD214:

1. DD214's can now be requested on line via the Internet. It takes about 
   ten (10) business days to receive them. 

   To order YOUR records online:
      vetrecs.archives.gov (requires JavaScript to be enabled) 

   Those applying online will receive priority over those who mail in the 
   SF-180 application.  

   The veteran has to be the one requesting. After the online application 
   is completed, a form will appear for printing. The veteran then signs 
   the form, and the form is either faxed or mailed to the records center 
   in St. Louis.  

2. Mail SF-180 to:
   National Personnel Records Center
   Military Personnel Records
   9700 Page Ave
   St. Louis, MO 63132-5100

3. Fax SF-180 to:  Fax number 1-314-801-9195

4. Call Fax-on-Demand System:
   Call 1-301-837-0990 from a fax machine, using the handset.  Follow the 
   voice instructions, and request document number 2255. 
   
       Get a copy of the Standard Form SF-180.
