The flag is to be flown at half-staff in the following circumstances:

Peace Officers' Memorial Day (May 15),
  unless that day is also Armed Forces Day (third Saturday)

Police Week (the week in which May 15 occurs)
  [ 36 U.S.C. 1 § 137 ¶ (2) ]
  The Presidential Proclamation typically requires half-staff display only
  on Peace Officers' Memorial Day (above), not during the entire week.

Memorial Day until noon (last Monday in May).

Patriot Day (September 11)
  [ 36 U.S.C. 1 § 144 ¶ b(2) ]

Fallen Firefighters Memorial Service (first Sunday in October, 10a-1:15p ET)
  [ 36 U.S.C. 1513 § 151302 Notes Pub. L. 107-51 (bottom of page) ]
  The Public Law 107-51, Oct. 16, 2001, 115 Stat. 267 requires half-staff
  only during the Memorial Service (broadcast live from Emmitsburg, MD),
  but on Sunday the flag is typically flown at half-staff all day. (The
International Association of Fire Fighters hold a service in September.)

National Pearl Harbor Remembrance Day (December 7)
  [ 36 U.S.C. 1 § 129 ¶ b(2) ]

By order of the President, the flag shall be flown at half-staff upon the
death of principal figures
of the United States Government...

By proclamation of the Governor of a State, territory, or possession.
